Employee Checklist

  • Time off requests are submitted to the Head of Staff. After review, your supervisor will communicate with you regarding the approval/denial status of your request.

  • Upon approval, your employee record will be updated, your requested dates will be added to the shared Outlook time off calendar, and your form will be filed.

  • Please personally inform those you work directly with of your upcoming time off. Preferably 2-3 weeks in advance if possible.

  • Employees are responsible for seeing the request through. Please keep an authorized copy for your records.
