Employee Checklist
Time off requests are submitted to the Head of Staff. After review, your supervisor will communicate with you regarding the approval/denial status of your request.
Upon approval, your employee record will be updated, your requested dates will be added to the shared Outlook time off calendar, and your form will be filed.
Please personally inform those you work directly with of your upcoming time off. Preferably 2-3 weeks in advance if possible.
Employees are responsible for seeing the request through. Please keep an authorized copy for your records.
